4. Provide your year of study, term or semester, and the specific form you are currently in.
5. List the total fees required for the year and the amount you are requesting. If applicable, include your student’s national ID and contact information.
6. Fill in your parent's or guardian's mobile number, along with other details such as their occupation and the total number of siblings in education.
7. Proceed to Part B: Family Information and tick the appropriate options regarding your parents' status and any disabilities.
8. In Part C, detail family financial status, including all siblings currently in educational institutions along with their fees and payment statuses.
9. Complete Part D: Declaration by signing and writing the date for both the student and the parent or guardian.
10. Collect and attach all required supporting documents, such as fee structures and identification, as outlined in Part D.
11. Once all sections are completed and documents attached, review the form for accuracy. You can then save your changes, download the completed form, or print it for submission.
