5. Next, denote the legal status of custody and guardianship by checking the corresponding box. Options include sending agency custody, parental custody, court jurisdiction, or parental rights terminated.
6. Proceed to Section III—Services Requested. Mark the box for the type of home study being requested, ensuring only one box is marked. Choose the type of supervision and how frequently supervisory reports are needed from the receiving state.
7. Provide the name and address of the supervising agency in the receiving state if known, or note that this will be completed later. Indicate what information accompanies the Placement Request by checking the relevant boxes.
8. Lastly, sign where indicated for both the sending agency and the sending state compact administrator. Ensure all required fields are completed before finalizing.
9. After filling out the form, you can save your changes, download, print, or share the form as needed.
