Donor history questionnaires (DHQs) are an important tool to ensure that donors are in good health and free from transfusion-transmitted infections.

Full-Length Blood Donor History Questionnaire (DHQ), Version 2.1 April 2020.

The DHQ includes not only all of the questions that must be asked of U.S. blood donors, but also flowcharts outlining the steps to take depending on a donor's answer to a question, educational materials for donors, and a list of medications that lead to donor deferral from donation.

Standard Laboratory Tests ABO typing (blood type) Rh typing (positive or negative Rh antigen) Screening for any unexpected red blood cell antibodies that may cause problems in the recipient. Screening for current or past infections, including the following: Hepatitis viruses B and C. Human immunodeficiency virus (HIV)

Screening Donated Blood If the donor's answers indicate they are not well or are at risk for having a disease transmissible by blood transfusion, they are not allowed to donate blood. If the donor is eligible to donate, the donated blood is tested for blood type (ABO group) and Rh type (positive or negative).

The purpose of donor screening and deferral procedures is to minimize the possibility of transmitting an infectious agent from a unit of donated blood to the recipient of that unit, as well as ensuring the welfare of the donor himself.

The purpose of donor selection is to assess the suitability of an individual to be a blood donor so that blood donation is safe for the donor and the blood products derived from this donation are safe for the recipients.
