3. In the 'Property Details' section, enter the address of the property for which you are requesting a valuation. Specify who can be contacted to gain access to the property.
4. Under 'Requirements', indicate whether the request is for a favourable or preferential purchase, and select whether it is a purchase or refinance.
5. Select the type of BMM product applicable, such as vacant land or construction, and include the loan ID if known.
6. If this request pertains to a property purchase, attach a copy of the contract for the property being purchased. If it is for refinancing, attach the most recent Rates Notice.
7. In the 'Payment Details' section, indicate your method of payment. Fill in the cardholder's name, credit/debit card number, expiry date, and the amount to be charged. Sign the form where indicated.
8. If using direct credit, ensure to include your surname as a payment reference.
