If you didn't receive a DD Form 214 when you left the service, you should contact your last unit if you separated recently. You can also contact your branch of service admin headquarters (Army Human Resources Command, Air Force Personnel Center, or Navy BUPERS).

The form no one tells you about is, in many cases, more important than the one everyone thinks they know (DD214). The DD 256 and 257 are issued when the person has not met the active duty requirements to be considered a veteran by the DoD. But having a DD214 form doesn't automatically mean you are a veteran!

Not every person associated with military service will have a DD214. If you're in the active or inactive reserves, you are technically not fully separated from your service. The same is true for anyone in the National Guard or those who haven't completed their service or been formerly discharged.

DD214 is a one-page document that contains information in relation to your service at the time of separation. This information is used to verify military service for any benefits, future employment, retirement, and membership in veteran organizations.

If you lose your DD form 214, you will need to request a replacement copy for your records. The DD214 is your most important post-military document. Treat it the way you would a marriage certificate, divorce decree, adoption papers or other important records.

A Report of Separation is generally issued when a service member performs active duty or at least 90 consecutive days of active duty training.

Visit the veterans page of Archives.gov (see Resources). At this website you can create your request form to receive the deceased veteran's DD214. Once the request form is created, you'll need to print it out, sign it and mail it. Click on “Request Military Records” at the bottom of the Archives.gov page.
