3. Provide details for the event you are participating in, including the MDA Muscle Walk location, event date, and your local MDA office.
4. Fill out the walker information section, including your name, email, phone numbers for day and evening contact, and your address (city, state, ZIP).
5. If applicable, check the option indicating whether your company has a matching gift program, and include related donor information as needed.
6. In the donor section, list each donor's name alongside their phone or email, along with the matching gift amount. Continue this for up to ten donors.
7. Calculate the total amount raised by adding all the donations and enter this in the provided total field.
8. Once completed, review your entries for accuracy. After confirming the details, you can choose to save changes, download a copy of the form, print it, or share it as needed.
