- Continue to Section 5, where you classify the nutritional status of children by weight for age. You will need to provide numbers for various age categories and their weight classifications.
- In Section 6, provide information regarding Nutrition and Health Education (NHED) activities organized and the participation numbers.
- Section 7 requires you to summarize the referral services provided during the month.
- Detail the distribution of iron and folic acid (IFA) tablets in Section 8, noting the total given to pregnant and lactating women.
- In Section 9, document the number of visits made to the AWC by the Auxiliary Nurse Midwife (ANM) and the Supervisor.
- For immunization details in Section 10, report the total number of children fully immunized by age group.
- Lastly, in Section 11, record information related to breastfeeding and weaning practices, including the number of newborns and breastfeeding statistics.
