- In Section 2, the healthcare provider must fill out relevant health information. This includes results from a preventative exam completed in the last 12 months. Ensure they provide all required metrics such as total cholesterol, blood pressure, HDL, body mass index, TC/HDL ratio, height, LDL, weight, triglycerides, waist circumference, and glucose.
- If applicable, the healthcare provider should apply their office stamp and provide their signature, name, phone number, and office address.
- Once both sections are complete, save your changes. You can then download, print, or share the completed Biometric Screening Results form as needed.

The results from the blood draw may take up to 10 days, depending upon the method. Most clinicians prefer to use a fingerstick to provide immediate results. The results from the screening may be used to identify specific health conditions. Your results will be available in your RealAge profile about 2 weeks after your screening.
