The Summary (300AP) — a separate form — shows the work-related injury and illness totals for the year in each category.

Private employers may obtain coverage by submitting a completed coverage application, Application for Ohio Workers Compensation Coverage (U-3) and a $120 security deposit. Public employers are unique in that they do not file for coverage in the same manner as private employers, such as filing a U-3 application.

PERRP helps ensure that Ohio public employees have a safe and healthful workplace. Public employers must provide a workplace free from hazards that are causing or may cause death or serious physical harm.
