7. Check all relevant medical history boxes if the patient has suffered from any predetermined illnesses. Provide additional details for any 'Yes' answers.
8. Indicate if the condition can be managed on an outpatient basis. Specify the required treatment plan, detailing any lab tests, imaging, medication, or procedures required.
9. Mention the estimated length of stay in days, as well as the estimated total cost in RM.
10. Finally, ensure the admission physician or surgeon signs the form and includes their stamp. If the admitting doctor is a medical officer, also include their name and specialty.
11. Once all fields are completed, you can save changes to the form, download a copy for your records, print the form, or share it as needed.
