5. In Section III, review the applicant certification statement. You will need to sign and date this section to confirm the accuracy of your information. If you are not the owner of the vessel, an owner or authorized individual must also complete and sign this part, providing their name, title, date, and contact information.
6. After thoroughly reviewing all entries to ensure accuracy, you can save the form, download it for your records, print it out for submission, or share it as necessary.
