5. Proceed to fill in the internship/shadowing experience information, including the advisor, department administrator, department, start and end dates of the internship/shadowing experience.
6. Answer the questions regarding laboratory work, technical experience, patient information access, clinical shadowing, and patient contact. Ensure compliance with the respective guidelines and policies for each situation.
7. Indicate the names and official university titles of any delegates who may supervise the intern.
8. Specify the educational goals of your internship or shadowing experience and indicate if the experience is solely internship or shadowing.
9. If it's an internship, describe the nature of the work and the type of final project that will be required.
10. Review your completed form to ensure all necessary information is accurate and complete. Once verified, email the completed form along with any required attachments to SMD-Internship-Program@urmc.rochester.edu.
