- Provide your physical address, including city, state, and zip code, which may differ from your mailing address.
- Enter your email address to receive notifications from the NICS Section.
- If different from the Licensee, provide the name of the point of contact person and their relation to the business.
- Fill in the point of contact phone number, specifying the telephone number the FBI will call when contacting your store.
- If applicable, enter the business fax number, including the area code. This is optional but may be useful for communication.
- Complete the section for FBI NICS E-Check users by providing the information of the employee who will access the E-Check.
- Include the email address for the employee who will be using the NICS E-Check, as a user name and password will be sent there.
- Read the NICS E-Check Subscribers Agreement carefully before signing the enrollment form.
- Sign the form and provide the date of execution. A witness must also sign and date the document.
