A drop of 10 mm Hg in the bottom number (diastolic blood pressure) within 2 to 5 minutes of standing also indicates orthostatic hypotension.

A decline of ≥20mm Hg in systolic or ≥10 mm Hg in diastolic blood pressure after 3 minutes of standing = orthostatic hypotension. A heart rate increase of at least 30 beats per minute after 3 minutes of standing may suggest hypovolemia, independent of whether the patient meets criteria for orthostatic hypotension.

In assessing orthostatic hypotension: The golden standard is the head‐up tilt test. Beat‐to‐beat blood pressure monitoring is essential for the diagnosis of orthostatic hypotension.

Orthostatic hypotension is defined as a decrease in systolic blood pressure of 20 mm Hg or a decrease in diastolic blood pressure of 10 mm Hg within three minutes of standing compared with blood pressure from the sitting or supine position.

Drinking 500 mL of water improved orthostatic tolerance by 5+/-1 minute (range, -1 to +11 minutes, P<0.001).

Clinically, a person is diagnosed with OH when systolic blood pressure (the top number in a blood pressure reading) drops by at least 20 mmHg during transition from lying down to standing up, or when diastolic blood pressure (the bottom reading) drops by at least 10mmHg within three minutes after standing.
