14. Complete the health condition section to disclose any relevant health issues the Uams Fitness Center staff should be aware of.
15. For students only, indicate your enrolled college and expected graduation year.
16. Read and understand the release and waiver of liability, and provide your signature and date at the end of the form.
17. Save your changes, and consider downloading, printing, or sharing the completed form as needed.
