- For clinic/support staff completion only, fill in the arrival time, end time, and total time. Ensure to comment if the total time exceeds 90 minutes.
- Once all sections are completed, submit the worksheet either via fax at 320-200-7980 or email the encrypted form to worksheets@archlanguage.com within 24 hours of the appointment.
