8. List any other income received over the last four weeks, including commission, work allowances, overtime, bonuses, and leave loading. Specify any income types not covered.
9. Review all entered information for accuracy and completeness before proceeding.
10. In the declaration section, the employer or company representative must sign, print their name, state their position, and enter the date. Additionally, they should provide a contact phone number.
11. Finally, save the changes, then download, print, or share the completed form as needed.
