7. Fill out the registered vehicle operator’s details and confirm if the vehicle is registered under a different name than the personalised plate owner.
8. If applicable, indicate if someone is lodging the application on your behalf and provide their details.
9. Complete the registered operator's declaration stating that the information provided is true. Both owners must sign in the case of joint names.
10. Review all information provided for accuracy. Once complete, you can save changes, print, or share the form as necessary.
