7. Continue by obtaining the signatures of the parent or guardian, head teacher, and the player in the designated areas. Each individual should also print their name and provide the date of their signature.
8. Move to Section B – Club Details. This section must be completed by the Club Secretary in BLOCK CAPITALS. Enter the secretary’s name and their current postal address. Provide an active email address and postcode.
9. Identify the recognized signatory, either the Club Secretary or equivalent, or the Chairman or equivalent. Enter the date for this section.
10. Ensure that all required sections are completed accurately. Once finished, you can save changes, download, print, or share the J15 Form as needed.
