The Shoulder Pain and Disability Index (SPADI) is a self-administered questionnaire that consists of two dimensions, one for pain and the other for functional activities. The pain dimension consists of five questions regarding the severity of an individual's pain.

The most used neck disability questionnaires are the Neck Pain and Disability scale (NPAD) and the Neck Disability Index (NDI). The Neck Pain and Disability scale (NPAD) is a composite index including 20 items which measure the intensity of neck pain and related disability.

The Neck Disability Index (NDI) is a self-report questionnaire used to determine how neck pain affects a patient's daily life and to assess the self-rated disability of patients with neck pain.

Score = 1 The pain is moderate at the moment. Score = 2 The pain is fairly severe at the moment. Score = 3 The pain is very severe at the moment. Score = 4 The pain is the worst imaginable at the moment.

The NDI can be scored as a raw scoreor doubled and expressed as a percent. Each section is scored on a 0 to 5 rating scale, in which zero means 'No pain' and 5 means 'Worst imaginable pain'. Points summed to a total score. The test can be interpreted as a raw score, with a maximum score of 50, or as a percentage.

The Neck Disability Index (NDI) was developed in the late 1980s by Dr. Howard Vernon and first published in the Journal of Manipulative and Physiological Therapeutics in 1991 (Vernon & Mior, 1991).

The NDI has become a standard instrument for measuring self-rated disability due to neck pain and is used by clinicians and researchers alike. Each of the 10 items is scored from 0 - 5. The maximum score is therefore 50. The obtained score can be multiplied by 2 to produce a percentage score.

The SPADI is a 13-item self-report questionnaire. The visual analogue scale is not commonly used. A minimum of 2/3 of items in each subscale must be answered in order to compute a subscale score. Total score is calculated by averaging the pain and disability subscale scores.
