9. Answer if the facility has a kitchen by marking Yes or No.
10. Enter your Reserve Center's FACILITY ID NUMBER as required.
11. In the section for NUMBER OF UNITS WITHIN THE FACILITY, enter the total number of units. Provide the names and UICs of the units, battle assembly weekends, and sources of procurement using the drop-down menus.
12. For JUSTIFICATION, briefly explain why subsistence cannot be obtained from another local Army Reserve unit or military dining.
13. Input the AKO user names, telephone numbers, and signatures for PRIMARY AND ALTERNATE AFMIS OPERATORS.
14. Complete the commander's name and signature section by entering the commander's AKO user name, signature, and date.
15. Once all fields are filled out, save your changes, download the form, or print it as needed.
