7. Fill out the 'Particulars of Person Who Makes the Payment (Head of Finance Department).' Enter the name, designation, P.O. Box, emirate, location, contact number, fax number, and email address.
8. Document the 'Bank References' by providing the name of the banks you deal with, the bank branch, account number, and telephone number for each bank.
9. Fill in the 'Trade Credit References' section by listing the names of companies you are dealing with on credit, along with the duration of the relationship and their contact numbers.
10. In the 'Documents Annexed' section, indicate which documents you are attaching, such as trade license copy, chamber of commerce certificate copy, or security cheque.
11. Carefully read and agree to the terms specified at the end of the form, ensuring to provide an authorized signature, date, your name, position, and company stamp.
