Follow the steps to effectively complete your certification form.

1. Press the ‘Get Form’ button to access the certification form and open it in the designated editor.
2. In the section regarding your certification on CEWs, check all applicable options for which you have received training, such as M26, X26, X26P, X2, or X3.
3. Clearly print your name in the designated field followed by your email address.
4. Fill in your complete address, including state and zip code.
5. Provide your agency name and contact phone number in the relevant fields.
6. Read the Product Warnings provided and, upon understanding them, sign your name in the Student Signature section.
7. If applicable, the instructor will complete the section designated for instructor verification, confirming that you have completed all necessary certification requirements.
8. Ensure that your instructor prints their name, signs the form, and provides the date, along with the location of training.
9. Once completed, keep the form for your department’s training records. Do not send this form to TASER Training.
