3. Fill in your husband's name, if applicable, and your current address details, including street or PO Box, city, state, and zip code.
4. Complete the section for your national number and the date you were admitted to the NSDAR, along with your state.
5. Detail your revolutionary ancestor in the appropriate space and list the state associated with that ancestor.
6. Provide the name of the chapter and district you belong to, and ensure you input your contact information, including telephone and email address.
7. In the relatives section, list qualifying relatives by providing their names, relationships to you, NSDAR numbers and states, years of membership, and whether they are living or deceased.
8. Complete one application form per member, making sure all necessary fields are filled with accurate information.
9. Once the form is completed, prepare to send it along with a check made payable to Treasurer Cameo Club, VADAR. Include the application fee and annual dues, totaling $10.00.
10. Send the completed application and payment to Emmalee Morris, Registrar, at the provided address.
11. After submitting, await your Cameo Club membership number. You may later purchase a Cameo Club pin, but remember you cannot wear it on your official ribbon.
