Follow the steps to complete your donation form online.

1. Press the ‘Get Form’ button to access the donation form in your online document management system.
2. Begin by entering your participant information. Fill in your first name, last name, address, city, province, postal code, phone number, email address, and team name clearly and accurately.
3. In the sponsor's section, provide the donor's name, address, city, postal code, and donation amount. Select the appropriate payment method: cash, cheque, or credit card. If using a credit card, enter the card number as indicated.
4. For donations over $10, check the box to request a tax receipt. Ensure all written information is legible to facilitate accurate receipt issuance for your donors.
5. Review all filled sections for any errors or omissions. Remember to photocopy the form for additional donations if needed.
6. Once you have completed the form, save your changes. You can then download or print the completed document as needed. It is recommended to submit the form by mail or in person at designated events.
