Authorization for Final Disposition Defined At it's simplest, the Authorization for Final Dispositions is a document set forth in the Wisconsin Statutes allowing Wisconsin residents to name who will be in control of their funeral arrangements and body disposition.

When a death occurs in Iowa and cremation is selected, it is required by law (Iowa Code 144C) that all providers get a written authorization from the legal living next of kin of the deceased. This form is called the Next of Kin Authorization.

Determination of legal next of kin uses the following order (if a person does not exist, is deceased, or is unable to be found, the next in line becomes next of kin): Spouse. Adult children. Parents.

1. A declaration shall name a designee who shall have the sole responsibility and discretion for making decisions concerning the final disposition of the declarant's remains and the ceremonies planned after the declarant's death.
