Follow the steps to successfully complete your timesheet online.

1. Press the ‘Get Form’ button to retrieve the timesheet and open it in the editing interface.
2. Fill in the facility name where you worked, as indicated in the designated box.
3. Input the pay period dates in the provided fields, specifying the start and end dates of the pay week.
4. Enter your name in the employee name section, ensuring it matches your official identification.
5. Provide the last four digits of your Social Security Number in the appropriate field.
6. Record the regular hours worked by indicating the time in and out for each day of the week on the timesheet.
7. Specify your meal times by filling in the start and stop times for the first and second meals.
8. If applicable, record any special pay hours or remarks related to on-call hours or callbacks.
9. Ensure that you total the hours worked for each day at the bottom of the timesheet.
10. Sign the timesheet at the designated spot to confirm that the information is accurate and that you received your meal and rest periods.
11. Have an authorized facility representative sign the timesheet and include their printed name and title.
12. Once completed, save your changes, download a copy for your records, and ensure the timesheet is submitted via fax to Emerald by the required deadline.
