- In Section 3, check all applicable comments from the officer about the detainee, including any issues such as language barriers or difficulty understanding questions. If other concerns arise, select 'Other' and provide details.
- Finally, follow the referral instructions. If the detainee answered 'Yes' to item 7, item 8, or at least two questions from items 1 through 6, they should be referred for further mental health evaluation. Indicate if the detainee is referred or not.
