4. In the 'Student Details' section, provide your full name, attach a recent photograph, and fill in your father’s name, mother’s name, and guardian’s name if applicable.
5. Indicate your category and input your date of birth and gender. Fill in details regarding your father's occupation.
6. For 'Matric /10TH Details & Aadhar Details', enter your Matric hall ticket number and passing year. Specify whether it was a regular or private examination.
7. Input your UID/EID number in the designated area and provide your permanent address, including block, district, and pin code.
8. Provide your communication address, ensuring to fill in the district, state, and pin code fields as well as your mobile number.
9. In the 'Institution & Course Details' section, enter the name of your institution, course, and the academic year of the course. Indicate if you are a day scholar or a hosteller.
10. If you are a hosteller, provide the name of the hostel and your hostel admission number. Also include your college admission number and the date of admission.
11. Fill in your bank details, including the name and district of the bank, branch name along with IFSC code, and your account number.
12. Read the student/guardian declaration, stating the accuracy of the information provided. Ensure you sign the declaration with the full address.
13. Finally, review all your entered information for accuracy, and save your changes. You can then download, print, or share the completed form as necessary.
