- Provide the name and address of the supplier in the respective field. If the goods are not manufactured by you, include the manufacturer's information here.
- State the reason for export, such as sale, loan, sample, or test, in the appropriate section.
- If applicable, include the harmonised tariff code for each item in the 'Harmonised Tariff' field.
- Indicate the country of manufacture in the designated field.
- Finally, certify that all information provided is correct by signing and dating the declaration, including your name printed clearly and your position in the company.
- Once you have completed all fields, ensure to save any changes to your document. You may also download, print, or share the completed form as needed.
