6. For both parts 3 and 4, proceed to fill out the table provided to request the service for specific categories or individual lines. Tick the relevant service categories in Table 1.
7. For individual lines, list the line numbers in Table 2 to ensure clarity regarding which services your billing preference applies to.
8. Once completed, review the form for accuracy, then save your changes, download, print, or share the form as needed.
