15. If your child is adopted, confirm if they are receiving a medical subsidy through the Adoption Subsidy Program.
16. Check the income bracket that corresponds to your taxable income as listed on your most recently filed tax form.
17. Confirm that your child lives in Michigan with one or both of the persons listed previously, marking YES or NO.
18. Read and understand all terms laid out, indicating your consent with your signature and the date.
19. Sign and date the application as the parent/guardian listed in step 10.
20. If applicable, have the other payee sign and provide their details in this section.
21. Once the form is completed, gather any required documents such as your Michigan income tax return, your child's birth certificate, and their social security card.
22. Submit the completed form and documents to the county community mental health services program representative that provided you with this form.
