Follow the steps to successfully complete your transcript request form.

1. Press the ‘Get Form’ button to access the transcript request form and open it for editing.
2. Enter your NPC Student ID and/or Date of Birth in the designated field to help identify your records.
3. Fill in today's date to indicate when the request is being made.
4. Provide your full name, including Last, First, Middle, and Maiden names, in the appropriate fields.
5. Input your current mailing address, including the city, state, and ZIP code for where the transcript will be sent.
6. Indicate the best way to contact you during business hours by providing either your phone number or email address.
7. Select if any notations are needed on the transcript and when the transcript should be processed (e.g., Send Now or Hold until degree is posted).
8. If applicable, indicate your Arizona General Education Curriculum status by checking the appropriate box.
9. Fill out the section for language proficiency if necessary, choosing between 2 or 4 semesters.
10. For each destination you want transcripts sent to, enter the full name and mailing address in the provided space.
11. Select your payment method by checking the appropriate box and filling in the required information.
12. Calculate your total fees and input the amounts for the Transcript Fee and any Overnight Fee as necessary.
13. Make sure to sign and date the form to authorize the release of your transcript.
14. Once completed, print the form and either mail, fax, or scan and email it to the Records & Registration office, or bring it to your local NPC office.
