1. Click ‘Get Form’ button to obtain the form and open it in the editor.
2. Enter your family name in the designated field at the top of the form. This identifies your order.
3. For each student in your family who is requesting pizza, complete a section of the form. Start by filling in the first name and room number for each student.
4. Select the type of pizza slices your students would like to order. Indicate the number of slices for Pepperoni, Cheese, and Ham & Pineapple by checking the boxes provided.
5. For the popcorn option, write the type of kernels (Caramel, Cheddar, or Butter Salt) next to the date you want the popcorn. Make sure to specify the date clearly.
6. Calculate the total for each student based on the selected items and enter this amount in the 'STUDENT TOTAL' section.
7. At the bottom, calculate the overall cost for your family and enter this amount in the 'FAMILY ORDER TOTAL' field.
8. Indicate whether you will be paying with cash or cheque. If using a cheque, ensure it is made payable to John Mahood PS and includes 'School Council pizza days' in the memo.
9. Once all sections are completed, review your order for accuracy and ensure all selections are correct.
10. Finally, save the changes, download, print, or share the completed form as necessary to submit it to the school before the deadline.
