7. Sign and date the form to confirm your agreement to the conditions of the offer.
8. Prepare your application for submission by attaching nine tokens and a self-addressed envelope (SAE) that is 15cm x 8cm with a first-class stamp attached.
9. Write your date band preferences on the front of your application envelope to ensure proper processing.
10. Submit your completed application to the specified address before the closing date of August 22, 2014, to finalize your booking.
