2. Begin by indicating if the horse is an NRHA Sire & Dam Offspring or if it is NRHA Futurity/Derby Nominated by checking the corresponding box.
3. Choose the horse's gender by marking one of the options: Mare, Stallion, or Gelding.
4. For non-registered horses, enter the color of the horse in the designated field.
5. Provide the registered name of the horse in the space provided, which will be used for licensing.
6. Submit photographs that show the full left and right sides of the horse, as well as the front and back views to highlight any marks of identification.
7. Fill in the breed of the horse, color, breed registration number, and breeder information in the respective fields.
8. If applicable, enter details for any additional breeds, including breed registration numbers and the country where the horse was foaled.
9. Enter the date the horse was foaled and provide details for the horse's sire and dam, including breed registration numbers.
10. List the current owner(s) of the horse, their NRHA Member ID number(s), address, contact number, and email information.
11. Ensure the owner(s) are current members of NRHA prior to submitting the application. Include a completed membership form if memberships have not been paid.
12. Select the payment option and fill in the required card details or indicate if you will be sending a check or money order.
13. If you require rush services for quicker processing, check the appropriate rush box and provide the email or fax number for confirmation. Choose either the 3 Day Rush or Same Day Rush.
14. Review all entered information for accuracy, and then save changes, download, print, or share the completed form as necessary.
