5. Indicate if the student is currently attending school and, if yes, provide the years attended and the name of the school. Specify if the student has been suspended or expelled, or if they are an F-1 or J-1 visa holder.
6. List the name of the responsible adult residing with the student, followed by their relationship to the student, such as mother, father, or guardian.
7. Complete the parent/guardian information section, providing the name, relationship, email, address (if different), employer details, and contact numbers.
8. If applicable, include sibling information by entering the names, birthdates, sex, schools, and grades of the student’s siblings.
9. Complete the medical/emergency information section stating names, relationships, and phone numbers of emergency contacts if neither parent/guardian can be reached.
10. Document any medical concerns, medications, or allergies the student may have.
11. Affirm the accuracy of the information by signing and dating the form at the bottom.
12. Finally, review the completed form for accuracy. You can then save any changes, download, print, or share the form as necessary.
