USCIS designates certain doctors (known as civil surgeons) to perform the immigration medical examination required for most green card applicants.

U.S. Immigration Medical Exam Public Health Criteria Form I-693 must be completed by the applicant and civil surgeon. Form I-693 must be signed by the civil surgeon within two years of filing Form I-485. Form I-693 must show that the applicant has received the required vaccinations (including the COVID-19 vaccine).

Introduction. Green Card applicants are required to complete a medical exam with an authorized civil surgeon. The standard exam does not include a drug test, but it involves urine and blood screenings that are known to catch certain drugs in your system.

They are licensed doctors who have at least four years of experience working in their chosen specialty. To become a civil surgeon, doctors must be certified by the United States Citizen and Immigration Services (USCIS). A civil surgeon may or may not specialize in performing surgery outside of immigration exams.

Rather, the notice is reminding you that you will need to submit a completed and signed Form I-693 Medical Exam to USCIS in the future, either: (1) at your adjustment of status interview; or (2) if requested by USCIS through a Request for Evidence.

The medical examination will include a medical history review, physical examination, chest X-ray and blood tests for syphilis. The physical examination will at least include examination of the eyes, ears, nose and throat, extremities, heart, lungs, abdomen, lymph nodes and skin.
