- For requests concerning program closure or loss of accreditation, describe the methods to help each applicant secure another training position.
- Add any additional information that may support your request, and be sure to submit any relevant supporting documentation to investigations@nrmp.org.
- Once all sections are completed, you can save your changes, download a copy, print the form for your records, or share it with the necessary parties.
