8. Review the Terms and Conditions at the bottom of the form. By submitting the form, you agree to extend your service contract with T-Mobile for 2 years on all lines in your account to receive the discount. Be aware that this request will be reviewed within 72 hours.
9. Once all submissions have been made, monitor your account, as the discount may take up to 2 monthly billing cycles to appear on your bill.
