EPA Standard Form 3520-1 – Declaration Form – Importation of Motor Vehicles and Motor Vehicle Engines Subject to Federal Air Pollution Standards. This form must be submitted to the U.S. Customs and Border Protection to import passenger vehicles, highway motorcycles and the corresponding engines into the United States.

If you were importing the goods for a garage that you work for or own, then the business could very well be the Importer and you, the Declarant. Either way, complete all boxes. Again, if it's a personal use shipment, your Declarant Capacity is simply yourself, or individual.

To import a motor vehicle you must file an HS-7 Declaration form (available at ports of entry or at http://.nhtsa.gov/cars/rules/import) at the time the vehicle is imported to identify the basis for the vehicle's entry into the United States.
