A DA 638 is a US Army form used to recommend a soldier for an award. This form is appropriate in most cases except for recommendations for awards for heroism, valor, or wartime, which require a separate process for recommendation.

1-1 Purpose: Soldier Readiness Processing (SRP) is a program within the United States Army, including its reserve components (the Army Reserves and Army National Guard), to qualify for pending deployments.

For most patients the SRP is done in two visits that are each about an hour long.

DA Form 5123–1 (In-Processing Personnel Record) guides a Soldier through the in-processing installation/community level program and DA Form 5123–1 provides a record of the Soldier's readiness status.

The Soldier Readiness Program (SRP) prepares, validates and reports individual Soldier and unit readiness for deployment to the unit commander.

A soldier may typically complete these stations in as little as 25 minutes or as long as two hours. The soldier will visit several stations during the administrative portion of the SRP, including legal, chaplain, life insurance, family situation changes, and security clearances.

Level III adds the movement of the unit's equipment and personnel off the installation via air or surface. An EDRE/SEDRE adds a no-notice element to a DRE, testing the Army's ability to rapidly deploy to support requirements unannounced.

Soldiers are provided pre- and post-deployment readiness, as well as general readiness, at the Soldier Readiness Processing (SRP). Service members and DoD civilians are given materials, guidance, and care to ensure deployments are executed and completed to the Army deployment standards.
