7. Complete the Utility Information by listing the city utilities you will connect to and confirming their availability by contacting the Utility Office.
8. Address the Required Documentation section. Mark yes or no for each required document, such as the site plan, building plans, and pre-manufactured data.
9. Provide the licensed plumber’s information if plumbing work is being contracted out. Also, indicate if your property is in a flood plain and provide the flood elevation if applicable.
10. Finally, obtain the necessary signatures of the landowner or contractor, then enter the date. Review all information for accuracy before proceeding.
11. Once you have completed the form, you can save changes, download, print, or share the application as needed.
