Many employers still require a “Courtesy Work Permit” form for summer employment, which certifies parental permission for summer employment. These forms are available from Division of the Registrar, 430 Cleveland Avenue, Columbus, OH 43215.

WORKING PERMITS: Every minor 14 through 17 years of age must have a working permit unless otherwise stated in Chapter 4109.

The minor will need to have a physical exam and have the licensed physician, physician's assistant or clinical nurse specialist complete the Physician's Certificate for Minor Work Permit, with which the doctor will certify that the minor is physically fit to work.

During summer months when school is not in session, 16 and 17 year old minors are not required to obtain work permits, provided that the employer maintains proof of age and a signed statement from their parent or guardian consent to their proposed employment.

Minors must be at least 14 years of age to obtain a work permit. Work permits can be deemed by the school superintendent. A new work permit must be issued each time the minor changes employment. Within 3 days of termination of employment the work permit shall be returned to the issuing authority.

To obtain a minor work permit, students/minors should use a pre-application form to gather the necessary information and approvals from their potential employer and from their parents or legal guardian.

The completed application should be submitted to the school in which the student is enrolled. If the student is enrolled in an alternative education setting (home schooled, GED) this application should be returned to the school district in which the student resides.
