4. List the high school level STEM courses you have completed or are currently enrolled in. For each course, input the course name along with the grade earned in the designated spaces. If necessary, add more rows for additional courses.
5. Complete the parent or guardian information section by providing their full name, relationship to the student, and their address. Fill in the contact email, ensuring that it's an email address you check regularly since all confirmations and communication will be sent there. Include a primary phone number and an optional second contact number.
6. In the payment information section, calculate the total amount based on your residency status and select your preferred payment method. If applicable, include any additional fees for transportation. Fill in the necessary fields for credit card payments, including your name on the card, card number, expiration date, and security code.
7. Review your application to ensure all information is accurate and complete. After verifying the details, save the changes made to your form.
8. Once satisfied with your application, you can then download, print, or share the completed form as needed. Make sure to send the form via mail or email by the specified deadline.
