- Review and confirm all data entered in the member acknowledgment and certification section. Ensure that you understand the implications of withdrawing your PERSI Base Plan funds.
- Sign and date the form. If you are married, your spouse must also acknowledge and sign the form, which requires notarization.
- Send the completed pages 1 and 2 of the form to PERSI at the provided address. Remember not to submit the form until after your termination date.
- After submission, you can save any changes, download a copy for your records, print the form, or share it as needed.
