2. Fill in the date field accurately to indicate when you are submitting the application.
3. Indicate your certification status by selecting one of the following options: 'New', 'Renewal', 'CSN' (Certified School Nurse), or 'NCSN' (National Certified School Nurse).
4. Provide your name as you would like it to appear in the records.
5. Enter your residential address in the designated fields including town, state, and zip code.
6. List the name of the certification authority from which you received your certification.
7. Fill in the date of issue for your certification.
8. Specify whether you wish to serve on the Executive Board by selecting 'Yes' or 'No'.
9. Provide your home email address and home phone number for contact purposes.
10. Select your educational status by indicating your employer, work address, and work phone number.
11. Input your work email address again for verification.
12. Select your field by indicating the grade level you are associated with: Pre-K, Elementary, Middle, or High School.
13. Choose whether your school is public or private.
14. Indicate your membership status by checking the appropriate box for Full/Active Member, Associate Member, or Retired Member.
15. If applicable, fill in the dues payment information, making a note that checks should be payable to BCSNA.
16. Finally, ensure that you click the 'submit via email' button to formally submit your application. The application will not be processed without this step.
