- Next, in the ‘Method of Payment’ section, select your preferred payment method (direct withdrawal, credit card, or check) and fill in the required payment details. Ensure you choose the payment frequency and include any additional processing fees applicable to your payment type.
- Finally, review the ‘Authorizing Signature’ section. Read the certification statement and provide your signature and date to authenticate the form. Remember, the form will not be processed without your signature.
