The Family and Medical Leave Act (FMLA) provides certain employees with up to 12 weeks of unpaid, job-protected leave per year. It also requires that their group health benefits be maintained during the leave.

The notice and leave request must: (1) be in writing; (2) refer to Merit Rule OAC 530:10-15-45; (3) describe the reason for the FMLA leave; (4) specify the type of leave to be charged during the absence; and.
