Follow the steps to complete the Online Isate Form.

- Begin by reviewing the Informed Consent section carefully. You will need to check one of the three options indicating your understanding and agreement to participate or not participate in the study.
- Fill in your name and signature in the designated fields for the client. Ensure that you also include the date of signing.
- If applicable, provide the signature of a witness, as well as a parent or guardian if the client is a minor. Remember to include the date next to each signature.
- Move on to the section for the intake staff. Accurate details such as the agency name, client case number, and agency code should be filled in appropriately.
- Continue completing the client’s personal details, including their social security number, recommended admission date, and the actual admission date.
- Next, fill out the intake assessment section, indicating whether it was completed by the health department and the recommended and actual ASAM levels of treatment.
- Provide relevant demographic information, including age category, gender, and ethnicity. Make sure to check the appropriate boxes.
- Please enter your home address and contact information. This includes a phone number for follow-up interviews.

You can take the ISAT as many times as needed, but it is recommended to aim for a maximum of two to three attempts for optimal results.
